چکیده
BACKGROUND Heart failure (HF) is mainly detected and managed in primary care, but the care is considered suboptimal. We present the rationale, design and baseline results of the Treatment Optimisation in Primary care of Heart failure in the Utrecht region (TOPHU) study. In this study we assess the effect of a single training of GPs in the pharmacological management of patients with HF. METHODS/DESIGN A cluster randomised controlled trial. Thirty primary care practices are randomly assigned to care as usual or intervention defined as a single training in the up-titration and management of HF drug therapy according to the heart failure guidelines of the European Society of Cardiology (ESC). Patients with a GP's diagnosis of HF will be re-evaluated by an expert panel of two cardiologists and a GP with expertise in HF to come to a definite diagnosis of HF according to the ESC heart failure guidelines. Those with definite HF will be analysed in this study. Drug use will be measured after six months, health status after twelve months, and heart-related hospital admissions and all-cause mortality after two years. DISCUSSION Our cluster randomised trial will show whether a single training of GPs improves the pharmacological management of patients with HF and confers beneficial effects on health status after one year, and cardiac hospital admissions and all-cause mortality after two years of follow-up. TRIAL REGISTRATION ClinicalTrials.gov Identifier NCT01662323.

The Effectiveness of Healthy Lifestyle Promotion Intervention on Quality of Life in Patients with Congestive Heart Failure via Cognitive-Behavioral Procedure
Introduction: Reduced quality of life in cardiac patients and their frequent hospitalizations in the coronary care units is regarded as a main challenge for such patients. Therefore, this study aimed to explore the effectiveness of healthy lifestyle promotion intervention on quality of life in patients with congestive heart failure via cognitive-behavioral procedure. Methods: In this randomi...
متن کاملHome-based exercise rehabilitation in addition to specialist heart failure nurse care: design, rationale and recruitment to the Birmingham Rehabilitation Uptake Maximisation study for patients with congestive heart failure (BRUM-CHF): a randomised controlled trial
BACKGROUND Exercise has been shown to be beneficial for selected patients with heart failure, but questions remain over its effectiveness, cost-effectiveness and uptake in a real world setting. This paper describes the design, rationale and recruitment for a randomised controlled trial that will explore the effectiveness and uptake of a predominantly home-based exercise rehabilitation programme...
متن کاملDesign and Evaluation of a Smartphone-Based Application to Manage the Treatment of People with Heart Failure
Abstract Aims: Patients with heart failure need training and support regarding the drug diet, how to use medications, diet, activities, and appropriate action when symptoms occur. The aim of this study was to design and evaluate a smartphone-based application to manage the treatment of people with heart failure. Materials and Methods: In this developmental-applied study with the software design...
متن کاملProphylactic Methylxanthines for Preventing Extubation Failure in the Preterm Neonates with the Gestational Age of ≤30 Weeks: A Randomized Controlled Trial
Background: Preterm neonates are at a high risk of respiratory depression at birth. Incidence of respiratory distress is reported in 60-80% of the neonates born with the gestational age of less than 28 weeks and 15-30% of the neonates with the gestational age of less than 32-34 weeks. The present study aimed to compare the incidence and risk of failed extubation in using caffeine and aminophyll...
متن کاملThe effects of curcumin on the prevention of atrial and ventricular arrhythmias and heart failure in patients with unstable angina: A randomized clinical trial
Objective: Inflammation along with oxidative stress has an important role in the pathophysiology of unstable angina which leads to acute myocardial infarction, arrhythmias and eventually heart failure. Curcumin has anti-inflammatory and anti-oxidant effects and thereby, it may reduce cardiovascular complications. This randomized controlled trial aimed to investigate the effects of curcumin on t...
